CANCELLATION POLICY
If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
PAYMENT OPTIONS
For your convenience, we accept cash, check, Visa, Master Card, American Express and Discover.
INSURANCE
We accept most major insurance carriers in the State of Hawaii including Workers’ Compensation insurance.

If it is not possible to complete these forms in advance, we will have them ready at the office.
